SUPPORT COORDINATION
Support Coordination is a service designed to assist individuals in navigating the National Disability Insurance Scheme (NDIS) and accessing the supports and services outlined in their NDIS plans. Support Coordinators work closely with participants to understand their goals, identify suitable service providers, coordinate appointments and supports, and ensure that their NDIS plan is implemented effectively. They provide guidance, information, and advocacy to empower participants to make informed decisions and maximize the benefits of their NDIS funding. Support Coordination aims to simplify the process, enhance participant choice and control, and facilitate a seamless and successful NDIS journey.
We offer these services locally to Sunshine Coast, Brisbane and the Gold Coast, further extending our services nationally, being contactable face to face, via phone call, email & via Microsoft Teams or Zoom.
We also offer Psychosocial Recovery Coach services to participants.
